I had the same problem. There seems to be a bug in BG3 that prevents you from using the an umbral gem on the pedestal if you used two in the first one. The problem has a simple solution - do not use the most intuitive way of putting the gem into the pedestal by: using the pedestal clicking the empty slot in the dialog
